Here’s the Scoop: The 5 W’s

WHAT: Sinai’s Ball Hockey Tournament – a 4-on-4 Round robin ball hockey tournament with a 3-game minimum, consisting of 12 teams in support of the hospitals greatest needs.

WHY: Mount Sinai is a small hospital specializing in respiratory, palliative and long-term care. Funds from the Ball Hockey Tournament will help support our hospital’s greatest needs.

Getting it Confirmed:

• Each team captain is responsible for securing their team sponsorship of $1,000.00.

• There is a mandatory registration donation of $50.00 per player which can be paid by each player or the sponsor can be invoiced an additional $500.00. Click Here to register!
• Both the team sponsorships and the registration donations are 100% tax receiptable.
• The deadline for team registration is August 30th, 2019, limited spaces available.
• Teams will not be confirmed until payment is received.
• Payment can be made via cheque, or credit card and a statement can be provided.

What you may be wondering:

• Players will be provided with: Team Shirts (which include the team sponsors logo), snacks, lunch and beverages.

• Each player is responsible for bringing their own stick and all other equipment. Balls will be provided.
